Kathmandu. Kathmandu: Negligence of air service providers and regulators was the main reason behind the plane crash at Tribhuvan International Airport in Kathmandu.
The final report prepared by the working group formed by the Ministry of Civil Aviation and Tourism under the leadership of Ratish Chandra Lal has pointed out human negligence as the reason for the plane crash.
The investigation committee has concluded that the plane crashed in a short time after leaving the runway.
The report pointed out the incorrect speed calculated on the basis of wrong speed card due to solar airlines plane crash, failure to identify, address many past incidents of high pitch rate while flying, and serious negligence of the air service provider.

1. Incorrect speed calculation based on wrong speedcard. For a total of 18,500 kg of take-off weight, the operator’s interpolated speed card mentions incorrect V speed for take-off. Although this error has already appeared in speedcard, it was ignored by the AIR service provider itself. There was no acceptance, approval of the speedcard booklet.
2. Failure to identify and address many previous incidents of high pitch rate during take-off by Sarya Airlines.
3. The operator showed gross negligence in following the prevailing practices of flight planning (ferry), preparation and implementation for maintenance purposes. There has been a lack of a consistent definition of flights when flying aircraft for maintenance.
4. Gross negligence by the air service provider in the entire process of cargo and goods handling (weighing, loading, distribution and latching). and non-compliance, while the provisions of the operating guidelines and ground handling guidelines have been violated. In addition, due to the haste of flight preparations, the cargo on the plane was not adequately protected with ropes, restraints or nets.
The ministry had formed a commission of inquiry headed by Lal, former general manager of civil aviation authority, comprising Dipuraj Jwarchan, Sanjay Adhikari, Sudip Bhattarai and Mukesh Dangol. The commission has prepared the report after a year-long study on accidents.
Eighteen people, including the crew, were killed when the CRJ200 aircraft crashed into the airport premises minutes after take-off on August 1, 2081. The relatives of the deceased are still knocking on the doors of the court to get compensation from the air service operator. They have not been compensated.
Issuing a statement on Friday, Prakash Khatiwada, who lost three children in the accident, lamented that the inquiry commission did not recommend action against those involved in negligence in the accident and compensation to the victim’s family. He said that 18 innocent people lost their lives due to serious negligence of Sourya Airlines, unforgivable mistake of the pilot and serious error of the regulatory body during the flight. The Commission could not fully expose it in truth. He demanded the government to form another inquiry commission. Khatiwada lost his daughter, son-in-law and grandson in the plane crash.
